When asciidoctor renders a manpage text to docbook (either docbook45 or docbook5), it doesn't set the source and the manual entries for refmiscinfo meta unlike asciidoc.

And the xmlto (at least the latest one on TW) mandates the source entry, and it fills "FIXME" if not defined.  This results in an ugly output "[FIXME: source]" in the footer, e.g.

The patch above adds the default entry of source for suppressing the FIXME.
It's the same value as asciidoc puts.

I'll try to do SR in the upstream github.

The fix was merged in upstream.
